To request an analysis, download and complete the Analysis Request form and the Customer Registration form (if you are a new customer or have updated your information). These forms will help you precisely define the analyses or studies you are requesting for your samples, as well as provide your complete and up-to-date identifying information.

This section also includes a list of sample acceptance criteria and a list of sampling representativeness criteria.
